Rising damp is simply water rising from the ground through capillary action,
The ground water also contains salts which also travels up through the wall.
The moisture from the ground water is absorbed through porous materials such as plaster and skirting boards.
Generally signs of rising damp include:

Paint
Paints begin to show signs of deterioration.

Wallpaper
Wallpaper begins to peel away from the wall.

Marks
Damp marks above skirting boards.
Causes
Rising Damp normally occurs due to a defective or non existent damp proof course.
Or in some instances where the damp proof course is “bridged “
Which allow the ground water to pass over the damp proof course because of a fault in the construction, For instance:
High external ground levels going above the damp proof course.
Internal plasters coming down past the damp proof course.
Treatments
The most efficient way to combat Rising damp is to make sure there is an effective waterproof barrier to stop any damp rising up the wall.
In existing buildings we do this by inserting a chemical damp proof course. After a wall has been treated with a chemical damp proof course it will slowly begin to dry out, but it is very likely that a wall effected by rising damp will have been contaminated by ground water salts, which will lead to a number of problems including efflorescence on the surface of the wall and decoration spoilage, for this reason it is often necessary to replaster the effected wall with suitable materials to Eradicate rising damp completely.
